Middle and High School Students'
Safe and Drug Free Schools Survey

During the month of January, Union County School System will have 6th-12th grade students participate in the Title IV Safe and Drug-Free Schools and Communities Program’s “Georgia Student Health Survey II”.  This is an anonymous, statewide survey tool that identifies safety and health issues that have a negative impact on student achievement. 

 

The survey covers various topics such as school climate and safety, school dropouts, alcohol and drug use, bullying, harassment, suicide, nutrition, sedentary behaviors, and teen driving laws. The survey contains approximately 100 questions and takes approximately 20 minutes for students to complete.  A copy of the Middle School and High School survey is available at the links below and at the appropriate school. 

 

Parents may remove their students from the survey by printing the “Passive Permission Letter” available at the link below, and returning the completed form to the front office of your child’s school by Wednesday, January 11th.  

 

The permission form only needs to be returned to the school, if you do not want your student to participate in the survey.
